Important information

Internet centers where you can fill out the Electronic Visa Application Form (EVAF) on your own

Applicants who don't have access to Internet may visit one of the free public Internet centers sponsored by the U.S. Government. The addresses may be found here: http://kyiv.usembassy.gov/irc_special_projects_eng.html.

Application download

Here you can download the application forms that are required to get a U.S. non-immigrant visa.

Applicants must sign the forms and bring them to their interview. The forms should not be sent to the Consular Section by any means.

DS-156 - This form must be filled out by every applicant who applies for a non-immigrant visa to the U.S. Please note that this form is not an appointment request. You have to send us an appointment request separately to be scheduled for a nonimmigrant visa interview.

DS-157 - This form must be filled out by any male applicant aged 16 to 45 (except applicants for the A, G, K and NATO visa categories).

DS-158 - This form must be filled out by any applicant requesting an F, J, or M visa.

Please ensure that the printed version of the application form contains a good quality bar code, as pictured below:
