We started filling out an application form DS-156, but then saw that it expired on September 30, 2007. Is there a new form available, or should we just use the current form on the website?

The document's expiration date is for internal use only. Applicants should continue using the form on the website. We will inform the public well in advance of any changes to application procedures.
