MEDICAL EMERGENCY

In case of medical emergency, applicants may schedule an emergency appointment by calling (38-044)490-4422.

At the time of their interview, applicants traveling to the U.S. on medical emergency must provide:

  1. A diagnosis from a Ukrainian physician stating the nature of the applicant's medical condition and why it can no longer be treated in Ukraine
  2. A letter from a U.S. physician stating that they are willing to treat the applicant’s condition. This letter must be faxed by the U.S. physician to the consular section.
  3. Estimate of the treatment's cost in the United States, provided by the U.S. physician or hospital
  4. Evidence that funds are available to pay for the treatment

NOTE: In case of death of the applicant’s close relative, a copy of death certificate should be faxed to (044) 484-42-56 prior to the interview.
