Changes of Name
- If you have changed your name, you may apply to have this
change reflected in your passport. Please note that as of
September 26, 2005, U.S. passports will no longer be amended to
change items appearing on the data page; you will therefore need
to apply for a new passport in your new name.
Changes of Name By Marriage
If you have changed your name by marriage, you may apply for a
new passport to be issued in your new name. If your passport was
issued more than 12 months ago, you should follow the instructions
for Passport
Renewal.
If your passport was issued within the last 12 months, you may
follow the instructions below.
Application Requirements
Please submit the following:
- Your current U.S. passport;
- Your original marriage certificate. If it was issued in a
language other than English, a certified translation should be
submitted;
- Passport
application form DS-5504: Form DS-5504 must be printed on
separate sheets of paper. Do NOT print this form on the front and
back of the same sheet;
- Two identical 5x5 cm passport photographs: The Embassy will
reject any photographs that do not meet the specified
requirements. Vending machine photographs and photographs
printed on a home computer are unacceptable;
- Fees: There is no fee for this service if your current U.S.
passport was issued within the last 12 months.
Processing Time
The application for a full-validity passport will be sent to the
United States for printing with a secure, photo-digitized image of
the bearer. You should allow seven working days for your application
to be processed. When your new passport is ready, you will need to
come back to the Consular Section of the U.S. Embassy to pick it up.
Your old passport must be submitted to the Consular Officer. It will
be cancelled and returned to you at this stage.
In cases where the applicant must travel urgently or has an
immediate need for a passport, the Embassy or Consulate can issue an
emergency
limited-validity passport, which cannot be extended.
Changes of Name By Means Other Than Marriage
If you have changed your name, you may apply for a new passport
to be issued in your new name. Under U.S. passport law, only certain
forms of name changes may be entered into a U.S. passport, and
application requirements vary depending on the way the name was
changed. Please read the Specific
Requirements for Changes of Name section for detailed
information.
Application requirements vary slightly depending on when your
most recent passport was issued. Please read the instructions
carefully, as incomplete documentation may delay your case.
Application Requirements
Please bring the following to your appointment:
- Your current U.S. passport;
- Passport
application form DS-5504: Parents should sign for children
under age 14. Form DS-5504 must be printed on separate sheets of
paper. Do NOT print this form on the front and back of the same
sheet;
- Additional documents as requested in Specific
Requirements for Changes of Name for your specific type
of name change;
- Two identical 5x5 cm passport photographs: The Embassy will
reject any photographs that do not meet the specified
requirements. Vending machine photographs and photographs
printed on a home computer are unacceptable;
- Fees: There is no fee for this service if your current
passport was issued within the last 12 months;
- If applying for a child under the age of 16, you will also be
required to submit the parental
consent requirement.
Additional Requirements for Passports Issued More than 12
Months Ago
In addition to the above listed requirements, please also provide
the following if your passport is more than one year old:
- Passport
application form DS-11 instead of form DS-5504: Applicants
aged 16 and over must sign their own applications. Form DS-11 must
be printed on separate sheets of paper. Do NOT print this form on
the front and back of the same sheet;
- Fees: The application fee for an applicant under the age of 16
is U.S. $85; aged 16 and over is $100. The accepted payment method
is cash, U.S. dollars only.
Method of Application
You are required to apply for the passport in person at this
office within American Citizens Services Unit public working hours.
